Location: St. Cloud, FL
Schedule: Monday-Friday, 8 am-5 pm
Work Setting: Onsite, outpatient clinical environment
Dress Code: Navy blue scrubs

Overview

We are seeking an experienced and patient-centered Case Manager (RN) to support individuals with high-risk health conditions and help reduce overall admissions, readmissions, and emergency room utilization. This role focuses on empowering patients through education, resource support, and advocacy while collaborating closely with a multidisciplinary care team.

Key Responsibilities

The Case Manager will have the following core duties:

  • Support patients with complex or high-risk medical conditions to effectively navigate the healthcare system.
  • Reduce avoidable hospital admissions, readmissions, and emergency department visits through proactive outreach and care coordination.
  • Educate patients and families on health conditions, treatment plans, medications, and self-management strategies.
  • Serve as an advocate and resource to enhance patient quality of life.
  • Collaborate with physicians, nurses, pharmacists, social workers, laboratory staff, and other allied health professionals.
  • Monitor patient progress, identify barriers to care, and address gaps in treatment.
  • Maintain accurate, timely documentation in accordance with organizational and regulatory standards.
  • Uphold HIPAA and patient confidentiality requirements.
  • Use medical equipment appropriately and learn equipment maintenance as needed.
Required Skills, Experience, and Attributes

Experience Requirements:

  • Prior healthcare experience in a hospital, urgent care, or primary care setting is required.
  • Medicare Advantage experience is preferred.

Skills & Competencies:

  • Strong verbal communication and interpersonal skills with the ability to interact professionally with patients and staff at all levels.
  • Ability to work independently with minimal supervision.
  • Effective multitasking skills in a fast-paced environment.
  • Strong organizational and time management abilities.
  • Proficiency with Microsoft Office, including Excel.
  • Working knowledge of medical terminology.
  • Knowledge of medical equipment and willingness to learn equipment maintenance.
  • Understanding of HIPAA regulations.
  • Ability to successfully complete a color blindness test.
Preferred Qualifications (Nice to Have)
  • Bilingual in English and Spanish
  • IV certification
Required Certifications
  • Active Registered Nurse (RN) license
  • Basic Life Support (BLS) certification
